Medetomidine Test Strips: Naloxone Can't Reverse It

Medetomidine is a veterinary sedative in the street opioid supply. It does not respond to naloxone, and standard strips miss it.

A Sedative That Is Not an Opioid

Medetomidine is a veterinary sedative. It is used to calm animals, not people. In the last couple of years, it has started showing up in the street drug supply, mixed into opioids and other powders across North America.

It is not an opioid. That matters a lot, because the drug that reverses opioid overdoses, naloxone, does nothing to reverse medetomidine.

The Danger

Medetomidine causes deep sedation and slows breathing, similar to an opioid overdose on the surface. Someone can look like they are overdosing on fentanyl, but naloxone will not bring them back, because the cause is different.

That is what makes it so risky. A responder gives naloxone, nothing happens, and they do not know why. Naloxone is still worth giving, because the person may have taken an opioid too. But medetomidine changes the picture.

Standard Strips Miss It

No. Fentanyl strips do not detect medetomidine. Xylazine strips detect xylazine, a different veterinary sedative, but not medetomidine.

Dedicated medetomidine test strips now exist. They work the same way as other strip tests. Dissolve a sample, dip, and read the result in a couple of minutes.

Testing for It

The method is the same as any other strip.

  1. Mix the bag first, because adulterants settle unevenly
  2. Dissolve a small, measured amount in water
  3. Dip the strip and wait the correct time
  4. Read the result at the mark

Do not test a tiny scoop from the top of the bag. That is how you miss a contaminant and get a false sense of safety.

Signs of an Overdose

Medetomidine looks a lot like an opioid overdose. Slow breathing, unresponsiveness, small pupils. But there is one tell that is different. Medetomidine often causes a very slow heart rate, lower than what a typical opioid overdose produces.

If someone is unresponsive, call emergency services immediately. Give naloxone, because opioids may be involved too. Start rescue breathing if they are not breathing well. Stay with them until help arrives. See our emergency response guide for the full steps.

The Bottom Line

The drug supply keeps changing, and medetomidine is the latest reason the old tests are not enough. If you use opioids or powders that could contain them, test for fentanyl, nitazenes, xylazine, and medetomidine. Four strips is not paranoia. It is what the current supply demands.
